Qualifications

Responsibilities

• Reviews the complete electronic and paper medical record of discharged patients.
• Assigns ICD-10-CM and CPT-4 codes from documentation.
• Abstracts coded data into the coding system with patient details.
• Uses Coding Clinic guidelines for ICD-10-CM.
• Sequences codes to determine appropriate selections.
• Prioritizes daily work for timely submission to billing.
• Participates in training on ICD-10-CM updates and guidelines.
• Identifies and resolves documentation or abstracting issues.
• Suggests process improvements for department efficiency.
• Creates and maintains ultrasound billing policy grid.
