Job summary

Galileo Health is seeking a Nurse Practitioner to support our Population Health team using phone and video-based care for post-hospital discharge follow-up. The role emphasizes patient engagement, care coordination, and a digital-first approach to improve outcomes for complex patients.

You will work in a 24/7 telefoundation with 4-hour shifts, weekends on a rotating schedule, and full-time benefits eligibility. Prior telehealth experience and a CT license are required.

Qualifications

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active CT state license to practice.
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
• Experience engaging Medicare and Medicaid populations.

Responsibilities

• Cover the Transitions of Care program needs.
• Conduct telephonic outreach and scheduled video/phone visits post-discharge.
• Perform medication reconciliation and order follow-up labs or meds as needed.
