Job summary

Galileo Health is seeking a Nurse Practitioner to join our Population Health team, delivering post-discharge follow-up through phone and video-based care. You will work in a digital-first environment, coordinating care and building trust with patients.

The role requires an active PA license, 5+ years in Primary Care or Geriatrics, and a strong aptitude for virtual care, with bilingual English/Spanish valued. Flexible scheduling and strong benefits accompany the role.

Qualifications

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active state licensure in PA, ability to obtain other licenses.
• Experience in or appetite for virtual care delivery preferred.

Responsibilities

• Provide telephonic and video-based follow-up care for patients recently discharged.
• Conduct medication reconciliation and symptom screening.
• Coordinate care across hospital, ED, pharmacies and community providers.
• Communicate Galileo care model and connect patients to needed services.
• Collaborate with nursing team and meet population health targets.
