Job summary

Galileo Health is seeking a Nurse Practitioner to join our Population Health team. In this role you will provide post-hospital discharge follow-up using phone and video-based care, delivering high-quality, comprehensive care to underserved patients and those with complex medical needs.

Cover transitions of care, perform medication reconciliation, coordinate with hospitals, pharmacies and community providers, and contribute to system-wide health outcomes while working in a digital-first

Qualifications

• Active PA licensure; NP certification in Adult/Family/Geriatrics.
• 5+ years of post-graduate clinical experience in primary care or geriatrics.
• Experience engaging Medicare/Medicaid populations with complex conditions.
• Comfort delivering virtual care and telehealth.
• Fluent in English; Spanish bilingual is a plus.

Responsibilities

• Cover Transitions of Care through telephonic outreach and video/phone visits.
• Perform medication reconciliation and arrange follow-up labs or prescriptions.
• Build trust with patients and explain Galileo's care model.
• Coordinate care across hospital, ED, pharmacies, and community providers.
• Collaborate with nursing team and meet population health targets.
