Job summary

Galileo Health, CT, is seeking a Nurse Practitioner to lead post-discharge follow-ups via telehealth and phone. You will engage patients with a patient-centered approach, reconciling medications, screening symptoms, and coordinating timely follow-up care.

You will work in a digital-first environment, collaborating with RNs and admins, and supporting a population health program focused on underserved, complex patients. Flexible scheduling and full benefits are offered.

Qualifications

• Certified Nurse Practitioner in Adult, Family or Geriatrics
• Active CT license with ability to obtain additional licenses
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Bilingual in English and Spanish is a strong bonus
• Comfort in a digital-first environment and virtual care delivery

Responsibilities

• Cover care needs for the Transitions of Care program.
• Telephonic outreach and scheduled video/phone visits for recently hospitalized patients.
• Medication reconciliation, symptom screening, and ordering follow-up labs or meds.
• Explain Galileo's care model and connect patients to services.
• Coordinate care across settings—hospital, ED, pharmacies, and community providers.
• Collaborate with the nursing team.
• Be accountable to individual and team population health targets.
