Job summary

Galileo Health seeks a culturally competent Nurse Practitioner to join our Population Health team. You will provide phone and video-based follow-up care for patients post-hospital discharge in a digital-first environment.

The role requires IL license, 5+ years of clinical experience in primary care or geriatrics, and the ability to work a minimum of 24 hours/week. Compensation is $75/hour base and $85/hour premium.

Qualifications

• Certified Nurse Practitioner in Adult, Family or Geriatrics.
• Active state licensure in IL, with the ability to obtain additional state licensures.
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics.
• 2 of those years may be substituted by time spent at a Skilled‑Nursing or Long Term Care facility.
• Experience engaging Medicare/Medicaid populations and managing complex conditions.
• Comfort in a digital‑first environment with virtual care delivery experience.

Responsibilities

• Cover care needs for the Transitions of Care program.
• Telephonic outreach and scheduled video/phone visits for recently hospitalized patients.
• Perform medication reconciliation, symptom screening, and coordinate follow-up labs or supplies.
• Navigate conversations to build trust and connect patients to Galileo services.
• Coordinate care across hospital/ED transitions, pharmacies, and community providers.
• Collaborate with the nursing team; contribute to population health goals.
