Job summary

The University of Miami/UHealth Department of Surgery Division of Plastic & Reconstructive Surgery has an exciting opportunity for a Full Time Nurse Navigator 1 to work in Miami.

The Nurse Navigator 1 serves as the initial point of contact for patients entering the health system and coordinates care after the first appointment, supporting patients throughout the care continuum and acting as a liaison among patients, families, caregivers, and the multidisciplinary team.

Qualifications

• Bachelor’s degree in relevant field.
• Valid State of Florida RN license.
• Minimum 2 years of relevant work experience.
• Ability to communicate effectively in both oral and written form.
• Proficiency in collecting, organizing and analyzing data.

Responsibilities

• Triages new patient appointments to ensure timely scheduling of initial appointments.
• Reviews outside medical records for scheduling suitability.
• Assesses barriers to care and refers to support services as needed.
• Educates patients on treatment plans based on diagnosis.
• Supports patients throughout the care continuum.
• Counsels individuals and patients on healthy practices.
• Collaborates with a multidisciplinary team to outline best treatment.
• Performs holistic evaluation of specialty populations to achieve better results.
