The Nurse Navigator 2 collaborates with a multidisciplinary team, educates patients about treatment plans and supports them through the care continuum, ensuring high-quality care and adherence to policies.

Qualifications

• Strong interpersonal skills and ability to communicate clearly with patients, families and teams.
• Ability to collect, organize and analyze patient data to support care planning.
• Experience in educating patients on diagnosis, treatment options and care plans.
• Competency with MS Office and electronic medical records.

Responsibilities

• Assess barriers to care and refer to support services, local and national organizations as needed.
• Educate patients on treatment plans based on their diagnosis.
• Support patients throughout the care continuum.
• Counsel individuals on positive health practices and lifestyle changes.
• Collaborate with a multidisciplinary team to outline best treatment approaches.
• Perform holistic evaluation of the patient population using advanced techniques.
• Contribute to improvements in patient care and healthcare policies.
• Mentor and coordinate preceptors for visiting professionals and students.
• Maintain professional knowledge via affiliations and continuing education.
