Responsibilities

• Performs care management within licensure scope for members with complex needs.
• Assess, develop, implement, coordinate, monitor and evaluate care plans.
• Provide care coordination telephonically or on-site for hospital discharge planning.
• Interface with Medical Directors and Physicians on treatment plans.
• Negotiate authorizations/referrals within benefits or extra-contractual arrangements.
