Job summary

Elevance Health is seeking a Nurse Case Manager I in Latham, NY, to perform care management for members with complex chronic needs, assessing, developing, implementing, coordinating, monitoring, and evaluating care plans. Roles may be telephonic or on-site, including discharge planning.

The role supports a hybrid/virtual work model with required in-person training, offers a Monday–Friday schedule with occasional evening shifts, and requires RN licensure and a BS/BA with 3 years clinical

Qualifications

• BA/BS in health field and 3 years clinical experience; RN license required.
• Multi-state licensure required if providing services in multiple states.
• Active RN license in applicable state(s).
• Case Manager certification is preferred.

Responsibilities

• Perform care management for members with complex chronic needs.
• Assess, develop, implement, coordinate, monitor, and evaluate care plans.
• Facilitate authorizations/referrals within benefits or through extra-contractual arrangements.
• Coordinate internal and external resources to meet identified needs.
• Monitor and modify care plans based on outcomes.
