Job summary

Elevance Health is seeking a Nurse Case Manager II (RN) for virtual, full-time care management with occasional in-person training. You will conduct telephonic assessments, develop and coordinate care plans, and work with Medical Directors to optimize outcomes for members with complex conditions.

The role requires an active RN license, multi-state licensure, and a BA/BS in a health-related field plus 5 years of clinical experience. Hybrid/remote options may apply depending on location and policy.

Qualifications

• Requires BA/BS in a health related field and minimum of 5 years of clinical experience or equivalent.
• Current, unrestricted RN license in applicable state(s) is required.
• Multi-state RN licensure is required for providing services in multiple states upon hire.

Responsibilities

• Conducts care management through screening, assessing, planning, implementing, and coordinating care.
• Outbounds: 100% telephonic care management and liaison with Medical Directors and other health professionals.
• Ensure member access to services and monitor effectiveness of care plans; adjust as needed.
