The Elevance Health Nurse Case Manager I role is a telephonic care management position designed to support members with complex, chronic care needs. The role is primarily remote, with in-person training sessions and potential alternate locations within commuting distance from an office.

The position emphasizes coordinating care plans, authorizations, referrals, and cross-functional collaboration with medical leadership to optimize member health outcomes.

Qualifications

• BA/BS in a health-related field required.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure if providing services in multiple states.

Responsibilities

• Assess, develop, implement, coordinate, monitor and evaluate care plans for members with complex needs.
• Perform duties telephonically to manage care across the continuum.
• Ensure member access to appropriate services and coordinate internal/external resources.
• Collaborate with Medical Directors and Physician Advisors on treatment plans.
