Elevance Health seeks a Nurse Care Manager I (NCLEX) to manage complex and chronic patient needs in a Puerto Rico setting. The role collaborates with physicians and care teams to ensure appropriate, high-quality care while coordinating with both English- and Spanish-speaking consumers.

Based in or near San Juan or Mayagüez, Puerto Rico, with virtual work options and required in-person training sessions. Responsibilities include assessing needs, planning care, and tracking outcomes to drive

Qualifications

• Requires a HS diploma or equivalent and a minimum of 3 years of acute care clinical experience.
• Active RN license in applicable state(s) required; multi-state licensure preferred for multi-state service.
• Bilingual in English and Spanish strongly preferred; NCLEX certification or active RN license highly preferred.

Responsabilités

• Ensure medically appropriate, high-quality, cost-effective care through assessing inpatient admissions and extensions of stay.
• Collaborate with providers to identify early discharge planning needs and develop care plans.
• Monitor and adjust care plans based on patient progress and outcomes.
