Qualifications

• Requires HS diploma or equivalent and 3 years acute care clinical experience; or equivalent background.
• Current, active RN license in applicable state(s).
• Multi-state licensure is required if providing services in multiple states.

Responsibilities

• Ensures medically appropriate, high-quality, cost-effective care through assessing inpatient admissions and extensions of stay, outpatient services, and level of care.
• Partners with physician reviewers/directors to interpret appropriateness of care and planning.
• Collaborates with providers to identify discharge needs and plan proactively.
• Conducts clinical assessments to develop goals and care plans.
• Monitors and evaluates the care management plan and updates as needed.
