Cardinal Health is hiring a Nurse, Individualized Care Nurse Care Management for a fully remote role. The nurse coordinates medical treatments, delivers patient education, and bridges gaps between patients, providers, and support systems to promote continuity of care.

The selected candidate will manage a caseload of 100–150 members, collaborate with onsite and remote teams, and ensure HIPAA-compliant documentation while supporting discharge planning and care transitions.

Competenze

• RN with California license (active, unrestricted).
• 5 years hospital or clinical experience in chronic disease care preferred.
• Care management experience preferred; oncology experience is a plus.
• Experience with Microsoft Office products.
• Excellent communication and organizational skills.

Mansioni

• Collaborate with health care staff to develop, implement, and monitor appropriate clinical care or other services to meet patient needs and coordinate all care activities.
• Strong knowledge of chronic disease management, including cancer.
• Ensure documentation compliance with federal, state, and regulatory agencies.
• Document all encounters and activities in the designated system accurately.
• Coordinate with oncology care and care management teams to assess and coordinate care.
• Assist with discharge planning and transition care management.
