Job summary

• Must have a nursing degree from an ACNE/CCNE accredited program.
• Indiana RN license active and in good standing.
• 3+ years nursing experience in population health, home health, ambulatory care, or case management.
• EPIC experience preferred; knowledge of teaching self-management preferred.

Responsibilities

• Coordinate care across multiple settings and providers.
• Support patients via telephonic interactions and occasional on-site visits as needed.
• Document care activities and outcomes in the EHR system.
