UnitedHealth Group seeks a Palliative Nurse Practitioner to deliver compassionate palliative care at home and in clinical settings. You will collaborate with physicians, build individualized care plans, and coordinate across care teams to ensure high-quality patient outcomes.

The role requires NP licensure, AANP/ANCC certification, DEA registration as needed, Medicare billing privileges, and 1+ years as NP or 3+ years as RN in geriatric, hospice, or palliative care.

Qualifications

• Master's degree or higher in Nursing.
• Licensed as a Nurse Practitioner in the state where services are provided.
• Certified as a Nurse Practitioner by AANP or ANCC.
• Authorized to practice and prescribe as an NP where permitted by law, with DEA registration as needed.
• Medicare billing privileges as an NP.
• Meets state-specific collaboration requirements and provides active collaboration agreements.
• Maintains state-required NP practice insurance or eligible for company policy.
• Current CPR certification.
• 1+ years as NP or 3+ years as RN in geriatric/hospice/palliative care.

Responsabilités

• Provides palliative care in home and clinical settings with physician collaboration.
• Performs Medicare-compliant assessments, schedules visits, and develops care plans.
• Orders and reviews treatments and tests; updates plans with physician input.
• Completes medical records promptly per state/federal rules.
• Coordinates care across providers and facilities; supports education and outreach.
• Educates patients and families about disease progression and treatment plans.
• Communicates progress and changes to collaborating physicians.
