Job summary

Local Infusion is seeking a licensed Nurse Practitioner (MA) to join a remote-first care team focused on neurology and memory care. You will deliver direct telehealth services to patients, develop and manage care plans, and collaborate with supervising physicians to improve access and reduce time to treatment.

The role requires an active MA NP license with geriatrics/AG or family practice preferred, 2+ years in neurology/memory care, and strong documentation and communication skills to support

Qualifications

• Active, unrestricted MA NP license; certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.

Responsibilities

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with our neurologist(s) and broader care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements.
