The Neuron Clinic, a busy outpatient neurology practice, seeks an infusion nurse with leadership abilities. The role focuses on operating CCM and PCM care programs, managing a patient panel, and monthly engagement per clinician plans of care.

Qualifications

• Minimum of one year of care management or case management experience.
• Minimum of one year of outpatient setting experience.
• Bilingual English/Spanish preferred.
• Current and unrestricted California LVN license preferred.

Responsibilities

• Maintain expert knowledge of care management scope and perform tasks accordingly.
• Maintain knowledge of available care management programs to perform required tasks.
• Collaborate with Referrals and Prior Authorizations to ensure patient orders are completed.
• Manage an active panel of complex neurological patients enrolled in care management.
