GLIA HEALTH MANAGEMENT LLC seeks an experienced infusion nurse with leadership abilities to join The Neuron Clinic network across CA. The role focuses on care management within CCM and PCM programs, coordinating patient panels and clinician plans of care. Experience in neurology or oncology is a plus.

The position supports flexible work-from-home options within California and offers competitive compensation and benefits. Strong communication in English/Spanish is preferred.

Qualifications

• A minimum of one year of care management experience is preferred.
• Experience in an outpatient setting is preferred.
• Bilingual English/Spanish is preferred.

Responsibilities

• Deliver care management to patients enrolled in CCM and PCM programs.
• Manage a panel of enrolled patients and engage with them monthly per plan of care.
• Coordinate with Referrals and Prior Authorizations to ensure orders are completed.
• Oversee care for patients with neuroimmunological disorders, Alzheimer's/dementia, movement disorders, and those receiving infusion therapy.
