Job summary

The Neuron Clinic in California is seeking an experienced infusion nurse with leadership abilities to oversee care management programs within our outpatient neurology practice.

This full-time role supports a panel of complex neurology patients enrolled in CCM and PCM, with monthly engagement according to each clinician's plan of care. Remote work from within California is supported and competitive benefits are offered.

Qualifications

• Minimum of 1 year in care management/case management.
• Minimum of 1 year in outpatient setting preferred.
• Bilingual English/Spanish preferred.
• CA LVN license preferred.

Responsibilities

• Maintain expert knowledge of care management scope of practice and perform tasks accordingly.
• Know The Neuron Clinic’s care management programs and perform required tasks.
• Coordinate with Referrals and Prior Authorizations to ensure patient orders are completed.
• Manage an active panel of complex neurological patients enrolled in care management.
