Job summary

The Neuron Clinic in California is seeking an experienced infusion nurse with leadership abilities to join our busy outpatient neurology practice. This full-time role focuses on care management within CCM and PCM programs, managing a patient panel and engaging monthly per clinicians’ plans.

Ideal candidates have 1+ year in care/case management and outpatient care, with bilingual English/Spanish preferred and an LVN license in CA.

Qualifications

• At least 1 year of care management or case management experience.
• At least 1 year of outpatient setting experience.
• Bilingual English/Spanish preferred.
• Current and unrestricted California LVN license preferred.

Responsibilities

• Maintain expert knowledge of the care management scope of practice and perform all tasks accordingly.
• Maintain knowledge of The Neuron Clinic’s care management programs to perform required tasks.
• Collaborate with Referrals and Prior Authorizations to ensure patient orders are completed successfully.
• Manage an active panel of complex neurological patients enrolled in care management, including those with neuroimmunological disorders, Alzheimer's/dementia, movement disorders, and those receiving infusion therapy.
