Job summary

The Neuron Clinic in California is seeking an experienced infusion nurse with leadership abilities to oversee chronic care and principal care management programs. You will manage an active panel of neurology patients and coordinate with referrals and authorizations to ensure timely infusion therapy and care planning.

This full-time, remote-enabled role offers competitive compensation, bilingual English/Spanish preferred, and opportunities for advancement within a busy outpatient neurology

Qualifications

• Minimum one year of experience in care management or case management.
• Minimum one year of experience in an outpatient setting.
• Bilingual English/Spanish preferred.
• Current and unrestricted California LVN license preferred.

Responsibilities

• Maintain expert knowledge of the care management scope of practice and perform tasks accordingly.
• Understand The Neuron Clinic’s care management programs and perform required tasks for each program.
• Collaborate with Referrals and Prior Authorizations to ensure orders are completed.
• Manage a panel of complex neurological patients enrolled in care management, including infusion therapy patients.
