Job summary

Vaya Health in North Carolina is seeking a PCS Assessor RN to perform needs-based assessments for Medicaid beneficiaries in home or supported living settings. The role includes scheduling, telephonic and face-to-face assessments, and submitting SARs for PCS providers.

Candidate must reside in NC or within 40 miles of the NC border and be willing to travel to designated counties. Active NC RN licensure and 2 years of home care experience preferred.

Qualifications

• Bachelor’s degree in Nursing and NC RN licensure required.
• Two years of home care experience preferred.
• Knowledge of NC Medicaid Personal Care Services settings.

Responsibilities

• Schedule assessments with members, families and providers.
• Conduct needs-based PCS assessments using standard tools.
• Interview family members and caregivers during assessments.
• Ensure assessments are completed within required timeframes.
• Submit SARs for PCS providers and coordinate with prescribers.
• Maintain privacy and dignity of beneficiaries at all times.
