Job summary

Vaya Partners seeks a Registered Nurse to perform needs-based assessments for Medicaid beneficiaries in home and community settings. The PCS Assessor will determine eligibility using standardized tools and coordinate service authorization requests (SARs) for PCS providers.

The role is remote but requires NC residency or within 40 miles of the NC border and travel to designated counties as needed. RN licensure in NC is mandatory; prior PCS assessment experience is preferred.

Qualifications

• Bachelor's degree in Nursing or another healthcare field required.
• Active RN licensure in North Carolina required.
• Two years in home care setting preferred.
• Experience with PCS assessments preferred.

Responsibilities

• Schedule assessments with members, families, and providers.
• Conduct needs-based assessments to determine PCS eligibility.
• Submit SARs on behalf of PCS providers.
• Maintain privacy and dignity of beneficiaries during assessments.
• Attend staff meetings and participate in case reviews.
