Job summary

CVS Health is seeking a licensed Pharmacist to deliver telephonic Medication Therapy Management (MTM) to high‑risk members. The MTM Pharmacist will perform comprehensive reviews, reconcile meds, document findings, and coordinate with prescribers and care teams.

This is a full‑time, remote role with training in behavioral interviewing and a focus on improving member outcomes. Candidates must have an active PA/IL/FL license, MTM experience and strong communication skills, and be able to work

Qualifications

• MTM experience with Outcomes MTM Connect platform.
• 7 months in MTM Ops with Meets Expectations in core metrics.
• Experience in a call center environment.
• Experience working from home.
• Excellent spoken and written communication skills.
• Proficient with Microsoft Office products.
• Strong prioritization and organizational skills.
• Active pharmacist license in PA/IL/FL.

Responsibilities

• Perform telephonic comprehensive medication reviews (CMRs) and complete medication reconciliations.
• Engage and HIPAA-affirm members for CMRs and document outcomes.
• Identify and resolve medication-related problems and create patient takeaways.
• Draft materials for prescriber offices to facilitate problem resolution.
• Maintain positive relationships with physicians, nurses, and internal teams.
• Complete internal training on behavioral modification and interviewing.
• Support patient engagement and CMR completion.
