TailorCare is seeking a licensed Doctor of Physical Therapy to deliver high-quality clinical navigation for members via telephonic and video interactions in a fast-paced virtual environment. The role requires engaging with members, assessing needs, and guiding them through evidence-based care plans.

The ideal candidate has at least 3 years in physical therapy or related clinical settings, strong communication skills, and comfort with technology-driven workflows.

Qualifications

• Doctor of Physical Therapy or equivalent.
• Current unrestricted PT license in applicable states.

Responsabilités

• Serve as the primary navigator for an assigned member population from intake through program completion.
• Deliver clinical navigation via telephonic and video-based interactions.
• Maintain high volume of member interactions while ensuring quality and empathy.
• Educate members on treatment options and support informed decisions.
• Collaborate with care teams to ensure coordinated member experience.
