Qualificações
- Licensed Doctor of Physical Therapy (DPT) with state license.
- Experience delivering care navigation in virtual/telehealth settings.
- Strong communication skills for patient engagement and education.
Responsabilidades
- Serve as primary navigator for assigned member population in a high-volume virtual care setting.
- Conduct clinical intake assessments for MSK conditions and readiness for care.
- Deliver navigation via phone and video, selecting optimal engagement modality.
- Maintain high volume of member interactions with empathetic, evidence-based approach.
- Educate members on treatment options and support adherence to care plans.
- Monitor progress and intervene when needed to optimize outcomes.
