Job summary

TailorCare, Inc. is seeking a licensed Doctor of Physical Therapy to serve as a Clinical Navigator.

You will lead member engagement via telehealth and phone, assess MSK needs, and guide patients through evidence-based care plans in a fast-paced, metrics-driven environment. Ideal candidates bring 3+ years in PT or musculoskeletal care, strong communication, and the ability to work across multiple digital platforms.

Qualifications

• Doctor of Physical Therapy or Master’s in Physical Therapy required.
• Current unrestricted Physical Therapy license in applicable states or ability to obtain.
• Minimum 3 years of PT/MSK care, care navigation, telehealth, or related clinical settings.

Responsibilities

• Serve as the primary navigator for a defined member population in a high-volume virtual and call-center style environment.
• Conduct comprehensive clinical intake assessments for MSK conditions and functional goals.
• Deliver navigation via telephonic and video-based interactions to support engagement and outcomes.
• Maintain high volume of member interactions with personalized, empathetic care.
• Educate members on treatment options and support adherence to care plans.
• Document interactions and ensure compliance with clinical protocols and standards.
