Job summary

Hennepin Healthcare is seeking a Mental Health Associate to join the Collaborative Care Model (CoCM) team as a Behavioral Health Clinician. This grant-funded role supports adults in a downtown primary care clinic, coordinating care with PCPs and a consulting psychiatrist.

Under clinical supervision, you will provide brief psychotherapy, participate in team rounds, and contribute to continuous program improvement to ensure long-term sustainability.

Qualifications

• Post Master’s graduate in Behavioral Health with supervision hours toward independent licensure (LICSW/LMFT/LPCC).
• Eligibility or licensure tracks available for LGSW, LPC, or LAMFT.
• Strong psychotherapy skills and ability to work within an integrated care team.

Responsibilities

• Participates in multidisciplinary care team rounds
• Provides mental health services including screening, diagnostic assessment, treatment planning updates, psychoeducation, psychotherapy, crisis services, referrals, and discharge planning
• Completes documentation for accurate and timely billing of clinical services
• Provides psychoeducation to patients, families, and community organizations as needed
• Provides care coordination including discharge/termination planning
• Provides on-call phone coverage for patient coaching as required
• May act as a preceptor to interns and students
• Maintains accreditation standards and state licensure/certification requirements
• Other duties as assigned
