Qualifications

• Master's degree in Psychology, Social Work, Counseling, or a similar field.
• Fully licensed mental health clinician preferred (LCSW/LCMHC/LPA/LMFT) or eligible.

Responsibilities

• Coordinate with DAC and LME/MCO for re-entry planning and services.
• Provide case management and referrals to housing, health care, and benefits.
• Travel within the community; field-based work and required documentation.
