Job summary

Qualifications

• Licensed professional (LP) in good standing
• Experience in care management or health services coordination
• Strong assessment and care planning skills
• Knowledge of HIPAA compliance and privacy practices
• Excellent communication and interpersonal skills
• Ability to conduct and document Health Risk Assessments (HRA)

Responsibilities

• Conduct Health Risk Assessments (HRA) and gather information on member health, needs, and preferences
• Develop and monitor person-centered Care Plans with specific services and goals
• Collaborate with interdisciplinary care teams (nurses, pharmacists, social workers) to coordinate care
• Ensure accurate documentation in the AHR and maintain compliance with state requirements
• Facilitate transitions and linkage to services, including LTSS, medical, and behavioral health supports
• Engage with members and families/caregivers through outreach and home/community visits
• Support crisis planning and continuity of care and assist with referrals and follow-up as needed
• Participate in routine team meetings and contribute to quality and safety initiatives
• Travel to member locations as required to deliver in-person care and assessments
