Job summary

Vaya Health is seeking a Care Manager to provide proactive intervention and coordination of care for members across mental health, substance use, developmental disabilities, traumatic brain injury, and LTSS. This mobile role requires NC residency and may involve travel to communities to ensure continuity of care.

You will perform HRAs, participate in multidisciplinary care planning, and link members to services while maintaining HIPAA-compliant documentation and coordinating with the care team

Qualifications

• Bachelor's degree required in health, psychology, sociology, social work, nursing, or related field.
• Two years of experience in behavioral health or LTSS settings.
• Strong communication and interpersonal skills.

Responsibilities

• Assess, plan, and coordinate care for members across MH/SU/IDD/TBI and LTSS.
• Perform Health Risk Assessments and maintain a comprehensive care plan.
• Collaborate with interdisciplinary teams and coordinate transitions.
• Ensure HIPAA compliance and effective documentation.
