Job summary

BlueCross BlueShield of Tennessee, Inc. is seeking a remote Social Work Case Manager to support elderly and disabled members through telephonic outreach, care coordination, and advocacy.

You will partner with members, providers, caregivers, and community organizations to address barriers to care and improve outcomes. Qualified candidates hold an active Tennessee Social Work license and have strong communication, organization, and coordination skills.

Qualifications

• Active Tennessee Social Work license required.
• 3 years of care coordination experience with long-term care or home and community based services preferred.
• Experience in SNF, acute inpatient settings, discharge planning, or hospice care preferred.

Responsibilities

• Provide crisis intervention and assist families in understanding medical situations and resources.
• Assess patient and family needs for health services on an ongoing basis.
• Interview patients and plan activities to meet social and emotional needs and support recovery.
• Coordinate with external agencies and home care teams for post-discharge care.
• Perform home visits as needed.
