Job summary

DaMar Staffing seeks a Medical Coder for a Veterans Affairs Medical Center role with responsibilities for assigning codes across inpatient and outpatient encounters. You will review records, ensure complete data capture, and support documentation and coding accuracy with ICD, CPT, and HCPCS.

Telework is available and the position operates under current Return-to-Office guidance. Schedule is 7:30am–4:00pm, Monday–Friday, with remote eligibility as described by agency policy.

Responsibilities

• Assigns codes to documented patient care encounters (inpatient or outpatient).
• Applies advanced knowledge of medical terminology, anatomy & physiology, disease processes, treatment modalities, diagnostic tests, medications, procedures as well as the principles and practices of health services and the organizational structure to ensure proper code selection.
• Selects and assigns codes from ICD, CPT, and HCPCS.
• Performs a comprehensive review of the patient health record to abstract medical, surgical, ancillary, demographic, social, and administrative data to ensure complete data capture.
• Assists facility staff with documentation requirements to reflect the patient care provided; provides technical support in regulations, coding requirements, reimbursement, and proper sequencing.
• Ensures provider documentation is complete and supports the diagnoses and procedures coded; consults with professional staff for clarification.
• Reports incorrect documentation or codes in the electronic health record.
• Searches the patient health record to justify code assignment based on organization and structure of the patient record.
