You will identify barriers and bridge clients to home visiting and early childhood services.

Qualifications

• Bachelor's or higher degree in Communications, Social Work, Information Technology, Education or related health/human services field.
• Knowledge of social service and medical providers for referrals.
• Excellent listening and judgement skills for assessing needs and referrals.
• Strong written and verbal communication; ability to work with diverse populations.

Responsibilities

• Handle a high volume of incoming/outgoing calls daily.
• Provide information, referrals, and resources to link clients to community services.
• Maintain filing system for cases and generate caller data reports.
• Collaborate with team and partners to ensure quality service and continuity of care.
