Job summary

Included Health is seeking a Maternity Nurse Care Manager to join our Care and Case Management team. This telephonic, remote role supports patients through pregnancy from preconception to postpartum with holistic, evidence-based care.

You will guide members, create care plans, and coordinate with multidisciplinary teams to ensure high-quality outcomes while navigating benefits and resources from a remote setting.

Qualifications

• BSN degree and active RN license.
• 5+ years of nursing experience including labor and delivery.
• 2+ years in care, case and disease management in health plans or TPAs.
• Experience working remotely and comfortable with technology.
• Strong verbal and written communication with empathetic patient care.

Responsibilities

• Deliver coordinated, patient-centered virtual care management by phone or messaging.
• Create personalized care plans with the care team and patients.
• Assist members in navigating benefits and healthcare systems.
• Collaborate with local providers for coordinated care.
• Provide longitudinal follow-up and compassionate support.
