Qualifications

• Active RN license in North Carolina.
• Must reside in North Carolina.
• 4+ years pediatric/maternity care or 2+ years in community-based care management.
• Willing to travel to visit Medicaid members in homes and clinics.
• Proficient with Microsoft Office.
• Spanish language skills preferred.

Responsibilities

• Assess, plan and implement individualized care management interventions.
• Identify referrals for healthcare and community-based services.
• Develop care plans as a single point of contact.
• Communicate health information to stakeholders for timely care.
• Advocate for members and families with the care team.
• Provide field visits up to 25%, within 120 miles round trip.
