Job summary

Included Health is seeking a Maternity Nurse Care Manager to guide members through the full spectrum of maternity care via telephonic and virtual modalities. You will collaborate with a multidisciplinary team to develop personalized care plans and support families from fertility planning through postpartum recovery.

The role emphasizes remote care delivery, HIPAA-compliant communication, and compassionate, patient-centric guidance to improve health outcomes for parents and their growing families.

Qualifications

• BSN degree required and RN license in good standing.
• 5+ years of nursing experience in maternity and/or care management.
• 2+ years in care, case and disease management, ideally in a health plan or TPA environment.
• 2+ years in labor and delivery.
• Active RN license in state of residence.
• CCM certification preferred.
• Ability to work remotely with strong technology skills.
• Empathy and patient-centric communication.

Responsibilities

• Deliver coordinated, patient-centered virtual care management by telephone or messaging.
• Create and refine care plans with the multidisciplinary team.
• Assist members in navigating complex medical conditions and benefits.
• Coordinate with members’ local providers for integrated care.
• Provide longitudinal follow-up and build supportive relationships.
