Job summary

Monogram Health, Inc. is looking for a Market Physician Executive (MPE) who will lead in-home multi-specialty care for chronically ill patients in Cape Coral, Florida. The MPE will be responsible for clinical strategies and oversight of daily operations while collaborating with various healthcare providers.

This role requires an MD or DO degree, strong leadership skills, and experience in renal care. Monogram Health is dedicated to improving health outcomes while providing comprehensive, personalized in-home care.

Qualifications

• Must obtain hospital privileges at required facilities.
• Active, unrestricted state medical license in each state within the market.
• Willingness to become licensed in multiple states.
• Experience with NCQA, HEDIS, Medicaid, Medicare.
• Current state medical license free of sanctions from Medicaid or Medicare.

Responsibilities

• Lead in-home multi-specialty polychronic care model in assigned market.
• Oversee daily clinical and business operations.
• Collaborate with community physicians and peer-to-peer decision making.
• Direct supervision of front line clinical and operations teams.

Skills

Management and communication skills

Experience in renal care and geriatrics

Evidence-based clinical criteria application

Experience with high-need Medicare Advantage

Education

MD or DO degree from an accredited medical school

BC or BE in an ACGME approved specialty
