Job summary

Monogram Health seeks a Market Physician Executive (MPE) to lead in-home multi-specialty care in an assigned market. The role oversees clinical and business operations, supporting APPs, RNs, SWs, and PharmD teams while collaborating with specialists to deliver high-quality care at home.

Ideal candidates have MD/DO, board certification in relevant specialties, and strong management and communication skills, with experience in renal care and population health.

Qualifications

• MD or DO with unrestricted license in market states.
• Experience applying evidence based clinical criteria.
• Renal care and geriatrics experience required.
• Willingness to obtain hospital privileges and multi-state licensing.
• Experience with Medicare Advantage and managed Medicaid.

Responsibilities

• Execute clinical guidelines and Monogram Model of Care.
• Lead operations to reduce cost of care and improve outcomes.
• Direct patient care oversight and care plan approvals.
• Guide Population Health Management and high-risk patients.
• Coordinate with community physicians and facilities.
