Monogram Health, Inc. is seeking a Market Physician Executive (MPE) to lead its in-home multi-specialty care model in Mississippi. The MPE will improve patient outcomes by overseeing clinical and business operations while collaborating with community providers.

The ideal candidate has a medical degree, leadership experience in clinical settings, and strong communication skills. They will be responsible for executing clinical guidelines and enhancing patient experiences through a comprehensive care approach.

Qualifications

• Demonstrated experience in evidence-based clinical management.
• Active, unrestricted state medical license required in each state within the market.
• Experience with high need Medicare Advantage and managed Medicaid populations.

Responsibilities

• Lead in-home multi-specialty polychronic care model.
• Oversee daily clinical and business operations.
• Collaborate with community physicians and facilities for patient care.
