Qualifications

• Willing and able to obtain hospital privileges at required facilities.
• Experience with NCQA, HEDIS, Medicaid, and quality improvement.
• Current state medical license free of sanctions from Medicaid or Medicare.

Responsibilities

• Lead in-home multi-specialty polychronic care model.
• Engage patients in treatment plans and community provider collaboration.
• Direct supervision of front-line clinical and operations team members.

Skills

Evidence-based clinical criteria

Education

MD or DO degree from an accredited medical school
