Job summary

Trillium Health Resources in North Carolina is seeking a Manager, Care Management – RCT Triage to provide strategic direction and oversee a team of care management professionals in a managed care environment.

You will guide workflows, ensure timely documentation, and collaborate with regional leadership to resolve referral concerns, while fostering a person-centered approach and delivering high-quality, integrated care.

Qualifications

• Associate's or Bachelor's in health/human services plus required experience.
• Must have valid driver's license and North Carolina residency.
• Legally authorized to work in the United States without sponsorship.
• Willingness to travel within catchment as required.

Responsibilities

• Manage care management teams and supervise staff.
• Ensure workflows for record reviews, screenings, and documentation.
• Maintain a team-based, person-centered approach to care coordination.
• Monitor metrics and quality indicators.
• Provide coaching and professional development to staff.
• Oversee referral intake, prioritization, routing, and assignment processes.
• Review referral trends and implement process improvements.
• Collaborate with regional leadership to resolve complex referral concerns.
