Job summary

Woodlands Primary Health Care is seeking a dedicated Medical Assistant to join our Chronic Care Management team. You will support doctors and patients, helping to develop care plans and educate patients on self-management.

The role focuses on chronic condition care, coordination of follow-up, and maintaining accurate electronic health records. Bilingual Spanish speakers are encouraged to apply; great teamwork and empathy are essential.

Qualifications

• Completion of a medical assistant program or demonstrated experience in medical field.
• Proficiency in medical terminology and EMR systems; eClinicalWorks experience preferred.
• Excellent communication and interpersonal skills.
• Team player with ability to multitask in a fast-paced clinical environment.

Responsibilities

• Collaborate with physicians to create care plans for patients with chronic conditions.
• Assist in patient education and self-management support.
• Coordinate follow-up appointments and ensure adherence to treatment plans.
• Monitor patient progress and communicate concerns to the healthcare team.
• Document patient encounters and maintain accurate records in the EMR system.
• Conduct medication reconciliation and assist with prescription refills.
• Perform basic patient assessments and vital sign measurements.
• Provide empathetic and compassionate care to patients with chronic conditions.
