Job summary

Phpmcs is seeking a Patient Care Coordinator (LVN) in a hybrid role primarily remote with periodic clinic work in Texas. You will coordinate care for Medicare patients, manage transitions of care, and engage high-risk individuals in self-management to reduce ED visits and hospitalizations.

The ideal candidate holds LVN licensure with 5+ years of medical office experience, and is proficient with EMR/EHR systems. Spanish bilingual abilities are preferred to aid diverse patient populations.

Qualifications

• Demonstrated ability to coordinate care for high-risk patients.
• Excellent communication with patients, families and clinical staff.
• Knowledge of HIPAA and patient privacy regulations.

Responsibilities

• Coordinate care management activities with clinical teams.
• Perform 48-hour follow-ups post hospital visits and document them.
• Identify gaps in care and schedule high-risk patient visits.
• Support transitions of care and patient self-management education.
• Maintain accurate documentation in the EMR
