Job summary

Success Matcher Recruitment seeks an LVN-led Patient Care Coordinator to guide patients through enrollment, onboarding and care navigation using AI tools. You’ll balance a hybrid in-office and remote schedule, with weekly outreach and close collaboration with nurses, providers and leadership.

Ideal candidates have 2–7 years in remote healthcare coordination, startup experience, and a strong tech mindset. Location near SF Bay Area or Sacramento is expected for monthly in-office days.

Qualifications

• Active LVN license (any state) for a non-clinical, care navigation role.
• 2–7 years of remote healthcare coordination or case management experience.
• Startup experience in healthcare or health tech with business acumen.
• Tech-forward mindset with interest in AI tools.
• Based in SF Bay Area or willing to commute from Sacramento; 6 days/month in SF office; 70% remote.

Responsibilities

• Patient Engagement: make 50–75 outbound calls weekly to enroll eligible patients.
• Caseload Management: onboard patients, verify eligibility, collect medical records.
• Care Coordination: ensure smooth handoffs between patients, nurses, providers and ops.
• AI & Process Innovation: help script AI workflows from patient interactions.
• Executive Collaboration: work with leadership to improve operations and metrics.
