Job summary

Comprehensive Rehab Consultants is seeking an LPN/LVN Care Coordinator to deliver Chronic Care Management remotely, supporting elderly patients toward desired health outcomes. You will conduct post-discharge outreach, develop patient-centered care plans, and coordinate services with CRC providers and the care team.

The role requires active LPN/LVN licensure, 2+ years in geriatric post-acute care, and 1+ year of 100% remote telephonic experience with direct patient interaction.

Qualifications

• Active LPN/LVN license or equivalent.
• 2+ years of geriatric post-acute clinical experience.
• 1+ years of 100% remote telephonic patient interaction.

Responsibilities

• Clinical Assessment & Planning: Review EMR records to assess physical and psychosocial risk and customize care plans.
• Care Transition & Monitoring: Track ED visits and post-discharge follow-ups with readmission risk assessments.
• Patient Engagement & Education: Use motivational interviewing for self-management and disease education.
• Care Coordination: Communicate findings, goals, and changes with providers, patients, and caregivers to obtain orders/referrals.
• Documentation & Compliance: Timely logging of plans, interventions, and outreach in care management software.
• Collaboration: Maintain strong working relationships with providers and care team.
