Job summary

UnitedHealthcare is seeking a LTSS Case Manager in Colorado to support the LTSS program by conducting intake, assessments, care planning, and ongoing case management. You will monitor health and welfare, document activities, and ensure confidentiality while coordinating with supervisors and CMA teams.

Requirements include a Bachelor's degree in human behavioral sciences or 5+ years LTSS experience, plus 1+ year MS Office proficiency and travel up to 50%.

Qualifications

• Bachelor's degree in human behavioral science fields (human services, nursing, social work, psychology, public health)
• 5+ years of relevant LTSS experience
• Combination of education and LTSS experience

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of member's health and welfare, documentation in required system
• Maintain confidentiality of patient information
• Conduct quarterly monitoring contacts and face-to-face visits at least every six months
• Complete annual reassessments and update care plans
• Monitor service delivery within the Support Plan and PAR
• Attend CMA agency training and in-service sessions
• Ensure work complies with CMA Contract requirements
• Refer suspected mistreatment to protective services or law enforcement
• May take on special assignments and system assessments

Education

Bachelor's degree in human behavioral science fields
