Qualifications

• Requires a Bachelor's degree and 2-4 years of related experience.

Responsibilities

• Evaluate member needs, resources, and coordinate care plan development for optimal outcomes.
• Assist in developing long-term care plans and identify providers and community resources.
• Coordinate between member/family and care team to ensure access to services.
• Provide resource support to members and families for housing, employment, and independent living needs.
• Monitor care plans and outcomes, and adjust recommendations as needed.
• Interact with long-term care providers to ensure needs are met.
• Collect and maintain member information to ensure regulatory compliance.
• May perform home or site visits to assess needs and collaborate with providers.
• Educate members and families on procedures, service options, and benefits.
• Provide leadership feedback to improve quality and cost-effectiveness of care.
• Performs other duties and complies with policies.
